Case Study 03

Patient’s initial problem:
ACL (Anterior Cruciate Ligament) tear and medial meniscus tear - sustained during a ski holiday

Pre-operative rehabilitation goals:

1. Reduce pain and swelling.

2. Restore range of motion.

3. Strengthen muscles around the knee.

4. Improve overall fitness and readiness for surgery.

Post-operative rehabilitation goals:

1. Control pain and inflammation.

2. Restore full range of motion.

3. Gradually increase weight-bearing.

4. Strengthen the knee and surrounding muscles.

5. Restore functional abilities and return to normal activities.

Background

Alice felt anxious about the upcoming surgery and was keen to minimise the recovery time. The pre-operative rehab helped in reducing pain and swelling, and Alice felt more confident going into surgery knowing his knee was in the best possible condition.

Treatment Details

- Cryo and Compression Therapy: Ice packs and compression boots to manage pain and reduce swelling.

- Range of Motion Exercises: Gentle stretching exercises and the use of a stationary bike.

- Strengthening Exercises: Quadriceps and hamstring strengthening using resistance bands and bodyweight exercises.

- Functional Training: Balance and proprioception exercises to prepare the knee for surgery.

Treatment Plan

  • Pain Management: Cryotherapy and prescribed pain medication.

Range of Motion: Passive knee extensions and heel slides.

Weight Bearing: Partial weight-bearing with crutches.

  • Range of Motion: Continued passive and active-assisted exercises.

Strengthening: Isometric exercises for the quadriceps and hamstrings.

Mobility: Gradual progression to full weight-bearing.

  • Strengthening: Leg press, step-ups, and resisted knee extensions.

Proprioception: Balance board and single-leg stance exercises.

Cardio: Low-impact activities such as swimming and cycling.

  • Strengthening: Advanced resistance training, including squats and lunges.

Functional Training: Agility drills, sport-specific drills, and plyometrics.

Return to Activity: Gradual return to running and recreational activities.

  • Sport-Specific Training: Intensive training mimicking skiing movements.

Functional Testing: Performance tests to assess readiness for return to skiing.

Progress and Outcomes

  • Alice was in considerable pain and worried about the long recovery process. She was uncertain about regaining his previous activity level.

  • Alice felt a mix of determination and frustration. The early stages were challenging, but the structured approach and consistent progress provided motivation. The support and encouragement from Powerbase Physiotherapy played a crucial role in maintaining a positive mindset.

  • After working with Powerbase Physiotherapy, Alice experienced significant improvements. She regained full range of motion, strength, and confidence in her knee. Alice successfully returned to skiing and other recreational activities, feeling stronger and more resilient than before.
