Case Study 04

Patient’s initial problem:
Herniated Disc

Patient’s goals:
1. Relief from pain

2. Return to activities

3. Improve work functionality

Background

Michael Thompson, a 34-year-old software engineer and recreational athlete, began experiencing sharp, radiating pain down his left leg after lifting a heavy box at work. The discomfort, accompanied by numbness in his foot, became progressively worse and significantly impacted his ability to sit for extended periods—a challenge in his desk-based job.

Michael also worried about his ability to continue running and playing soccer, activities he enjoyed on weekends. Despite trying to manage the pain with rest and online research, his symptoms persisted, leading him to seek professional help at Powerbase Physiotherapy.

Initial Assessment

During his initial visit, Michael described his pain as sharp and radiating, with numbness in his left foot. He rated the pain as severe, significantly impacting his daily activities and ability to sit for long periods. A thorough assessment conducted by the physiotherapist included a detailed medical history, physical examination, and postural analysis.

Key findings revealed a herniated disc at the L4-L5 level, poor core stability, and postural imbalances exacerbating the condition. The assessment provided a clear roadmap for a tailored rehabilitation programme designed to address his pain and functional limitations.

Treatment Plan

  • Comprehensive evaluation.

    Pain and function assessment.

    Treatment goals established.

  • The PT employed techniques to mobilize the spine and reduce muscle tension.

  • Michael worked on core stability exercises to support his spine, gradually increasing in difficulty.

  • The PT educated Michael on body mechanics and posture, helping him understand how to avoid aggravating his injury.

  • Techniques such as ultrasound therapy and electrical stimulation were used to alleviate pain and inflammation.

Progress and Outcomes

  • Michael felt overwhelmed and anxious about his herniated disc. The persistent pain and numbness in his leg made it difficult to focus at work and participate in activities he loved, such as running and soccer. He worried about the long-term effects on his physical health and his ability to maintain an active lifestyle. As the discomfort grew, so did his frustration, leaving him desperate for a solution.

  • During treatment, Michael noticed a gradual decrease in pain and improved mobility in his lower back and leg. He valued the tailored approach and the detailed guidance provided by his physiotherapist, which gave him confidence in the process. While some exercises were challenging, Michael found them empowering and stayed committed to his home exercise programme. Regular follow-up sessions allowed for adjustments to his treatment plan, keeping his recovery on track and progressively building his strength and resilience.

  • After completing twelve weeks of physiotherapy, Michael experienced significant improvements. His pain levels had greatly reduced, and he was able to sit comfortably for extended periods, return to running, and play soccer without discomfort. Michael felt more confident in his physical health, equipped with a deeper understanding of his body and strategies to prevent future injuries. The tailored treatment not only restored his mobility but also empowered him to maintain his progress through regular exercises and lifestyle adjustments.
